Is it possible in Buzz to do a survey question? I would like to ask a question like how ready do you feel for the final and then give 4 choices. None would be a "right" answer, but it would be a way to find out how the content is working. I as hoping to make this part of an assessment, but I am not sure how to do that.

Non-graded questions on assessments has been requested for some time (see this post, for example). I hope this becomes an option soon!

In the meantime, I've found that Google Forms are pretty easy to work with, and you can even embed them in a Buzz activity page.

There isn't a good way to do this at this time other than what has already been suggested. 

Not ideal, you could possibly use a non-gradable assessment with a Multiple Answer type question to fulfill this. The questions would be worth 0 points and each response would be set as the correct answer. Then set the assessment review settings to hide the scores from students as it could cause confusion to see responses marked wrong. The teacher can go review the responses then.
