We have a teacher who has a folder and some content in other folders missing when viewed in the student app. The folder and content does show in the teacher app and is not hidden on the Edit page. I checked the Visibility and access settings as well and verified that it is set to be viewable by teachers, students and observers. I also tried changing those setting to teachers only and then back, no change.

It sounds like from what you have described the folder and items within are configured to not be hidden to students. Is the folder and items within hidden from the table of contents? That setting is housed under the Visibility and access section of both folder and item settings. If it's not set to hide for students, I recommend that a ticket be submitted to support to further address this behavior.
